This one's from Grey's Anatomy's season 3 finale: "Didn't We Almost Have it All?" The song is "Keep Breathing" by Ingrid Michaelson.

The song plays over the end of the episode as Alex looks for Ava, Meredith helps Cristina rip off her wedding dress after being left at the alter, Callie watches Izzie, George meets Lexie, and Derek declines the Cheif's position, talking Richard into staying. The Chief is still the Chief.
