Sam institutes a schedule dot board to see when everyone is free to help with other activities, and this just makes those without appointments feel worse. To make everything even better, a couple comes to Addison for fertility help, all they’ve ever wanted was to have kids—they’ve got the nursery decorated and everything—and, after running some tests, she finds out that they’re actually brother and sister. They of course didn’t know this (children of the same sperm donor), and after dealing with the shock, decide they’re soulmates and still want to stay together. Addison refuses to do the procedure, and they insist that they don’t care that they’re siblings, the guy lets slip that he didn’t care when he found out, and suddenly his wife/sister has a complete change of heart. He knew? She tells him she never wants to see him again and cries in Addison’s arms.
Sam has to deal with a 17-year-old who is sick of living with a chronic lung condition. As far as he’s concerned, it hurts too much with all the tests and the machine breathing for him. He just wants his father to let him go. Since he’s not 18 yet, Sam has to do what his father wants, and ties the poor kid down to insert tubes so he can breathe. Both Sam and the man’s son eventually get him to realize that although he’s alive, he’s not living. The father finally gives in and lets his son pass quietly into peace.
In the end, Cooper finally goes to Violet for help in getting out his lice (seeing as how Charlotte would probably kill him if he approached her with lice-head) and ends up telling Violet the truth. Charlotte made him promise not to tell her, but he hates lying to her and now that she knows already he tells her.
Pete gets rid of the fat-sucking machine because none of his patients want to live a healthier life with it around, and Sam gets rid of the scheduling dot board because it’s not helping any of the doctors get new patients anyway. There’s still no Dell in sight, but the new temp sure has a thing for Pete. They put the massage table to good use, and in her words, she didn’t know “old guys” could be so good.
Naomi shows up at Sam’s even though she’s still furious at him.
Addison finally can't stand that her SWAT date didn't call her back, and calls him, leaving him a very long, babbling, embarrassing voicemail. She sighs and hangs up. What a day.
